I am an Adopted Person and:

Non-Identifying Information
- This is a summary of the information from your adoption file potentially including a medical history, social history of your birth parent(s) (birth family composition, physical descriptions, characteristics, occupations, etc), information about your birth, and any information about your foster care if relevant. Also, your birth name if you were named at birth (first and middle only).
- This will NOT give you any identifying information about who your birth family members are.
- You must be at least 18 years of age to make this request on your own.
- Please read and submit the Client's Rights form along with your request for any services.
- There is a $35 fee for this service. If this is something you cannot afford, we do provide free services based on financial need. Please fill out the Request for Financial Assistance form and submit it along with your request.
I was adopted in or before 1941 and I would like the identifying information from my adoption file
- Prior to 1941 in the State of Georgia, adoption records were not sealed. If you are an adult adopted in Georgia prior to 1941 you can receive identifying information from your adoption file, providing we are able to locate a file. Prior to 1941, adoption records were retained by the Superior Court in the county where the adoption petition was filed and finalized. This generally would be the county of residence of the adoptive parents at the time of the adoption. However, some finalized adoption records were submitted to the State Office of Adoptions. We have some records, primarily agency records prior to 1941. For other files, the court where the adoption took place would need to be contacted. There is no fee for this service
- You may also want to try contacting Vital Records
- Please read and submit the Client's Rights form along with your request for any services.
Search Request
- You must be at least 21 years of age in order to request a search for your birth parent.
- If you choose to start with intermediary contact, your identifying information will not be shared.
