Request to Contact Biological Family
If you are an adopted person over the age of 21 and you
have already received your non-identifying information, you may
then request that a search be done for one or both of your birth
parents. There is a separate fee for each birth parent
search.
Please complete the “Request to Contact
Biological Parent” form and the "Adopted Person Consent to Contact" form. When completing the request form, there is a space for you to list questions you may have for your birth parent. Please write these questions in the first person, as if you were speaking directly to your birth parent. You must specify which birth parent you wish to search for. Also, in addition to these forms, please include a letter (also written in the first person) to your birth parent including the following information:
- A physical description of yourself
- Your current family situation
- Your occupation
- Your motivation for searching
- What do you hope to gain from doing a search and making contact?
- What are your expectations?
- Provide some information about your relationship with your adoptive parents and other family members. Are they aware that you are conducting a search?
- How long have you known you were adopted?
- Any other information you would like to share with your birth family member
Please return these forms, once the Consent to Contact form is notarized, along with a $300 payment via check or money order payable to Families First to:

We also accept Visa or Mastercard payments via telephone. Once we have received your request form for a search, along with your payment, your search will be initiated as soon as possible. Most searches are completed within 1-6 months. Please be aware that the fee for conducting a search for a birth parent is non-refundable, regardless of the outcome.
Once your birth parent is contacted, we will notify you of his/her decision regarding contact. An adult adopted after 1941, may receive identifying information only with the written consent of the person being sought. Any person being sought must sign a “Consent to Contact” form before the Registry can release any identifying information.
The Registry can also release identifying information if the person requesting the information has a court order to open the adoption record. Contact the Registry to learn more about this process.
