Adopted Person Request to Contact Biological Sibling Who was NOT Placed for Adoption

If you are an adopted person requesting a search for your biological sibling who was not placed for adoption, the following requirements apply:

You must be 21 years of age and the sibling for whom you are searching must be at least 18 years of age. 

Complete the “Registration and Request to Contact Biological Sibling” form and the “Adopted Person Consent to Contact” form. When completing the request form, there is a space for you to list questions you may have for your sibling. Please write these questions in the first person, as if you were speaking directly to your sibling. You must specify which sibling you wish to search for. Also, in addition to these forms, please include a letter (also written in the first person) to your sibling including the following information:

  • A physical description of yourself
  • Your current family situation (married, single, children, etc)
  • Your occupation
  • Your motivation for searching
  • What do you hope to gain from searching and making contact?
  • What are your expectations?
  • Provide some information about your relationship with your parents and other family members. Are they aware that you are conducting a search?
  • Any other information you wish to include about yourself

Please return these forms, once the consent to contact form is notarized, along with a $300 payment via check or money order made payable to FAMILIES FIRST to:

We also accept Visa/Mastercard payments via telephone. Once we have received your request form for a search, along with your payment, your search will be initiated as soon as possible. Most searches are completed within 1-6 months. Please be aware that the fee for conducting a search is non-refundable, regardless of the outcome.

Once your sibling is contacted, we will notify you of his/her decision regarding contact. An adult adopted after 1941, may receive indentifying information only with the written consent of the person being sought. Any person being sought must sign a “Consent to Contact” form before the Registry can release any identifying information.
