Registration / Consent to Contact
If you are a birth parent and your child's adoption was finalized in the state of Georgia you may file a Consent to Contact Form with the Georgia Adoption Reunion Registry.
Please remember to notarize this registration form before you return it to us. Once this form is on file we will be able to release your name and address to the child, age twenty-one or older, upon his/her request. Georgia adoption records have been sealed since 1941 and identifying information from the record can only be provided to the adoptee.
