I am the child of a deceased adopted person:

I want to request information from my parent's adoption record »

I would like to request his/her non-identifying information

    • This is a summary of the information from your parent's adoption file potentially including A medical history, social history (birth family composition, physical descriptions, characteristics, occupations, etc), information about the adopted person's birth, and any information about the adopted person if and while they were in foster care. Also, the birth name if named at birth (first and middle only).
    • This will not give you any identifying information about who your parent's birth family members are.
    • There is a $35 fee for this service. If this is something you cannot afford, we do provide free services based on financial need. Please fill out the Request for Financial Assistance form and submit it along with your request.

I would like to request the identifying information from his/her pre-1941 adoption record.

  • Prior to 1941 in the State of Georgia, adoption records were not sealed. If you are an adult adopted in Georgia prior to 1941 you can receive identifying information from your adoption file, providing we are able to locate a file. Prior to 1941, adoption records were retained by the Superior Court in the county where the adoption petition was filed and finalized. This generally would be the county of residence of the adoptive parents at the time of the adoption. However, some finalized adoption records were submitted to the State Office of Adoptions. We have some records, primarily agency records prior to 1941. For other files, the court where the adoption took place would need to be contacted. There is no fee for this service.
  • You may also want to try contacting Vital Records.

  • Please read and submit the Client's Rights form along with your request for any services.
