Services Offered to Parents or Siblings of Deceased Birth Parents
Registration and Request to Contact Adult Adopted Person
If you are a parent or sibling of a deceased birth parent you may register and request to contact the adult adopted person, provided that the adult adopted person is over the age of 21 years, and the adoption was finalized in Georgia. You must provide documentation that the birth parent is deceased.
Please complete the "Registration and Request to Contact Adult Adopted Person" form. In the bottom section, please write in the first person, as if you were speaking directly to your biological family member. Also, make sure to include the following information:
- A physical description of yourself and the birth parent
- Your current family situation
- Your occupation
- Your motivation for searching
- What you hope to gain from doing a search and making contact
- Your expectations
- Information about your relationship with your family members
- Whether or not your family is aware that you are conducting a search
- Any other information you would like to share with your biological family member
If you need more space, please feel free to write a letter of introduction on a separate sheet of paper. Please return this form with a payment of $300 via check or money order payable to Families First or Visa/Mastercard.
Once we have received your registration form and your request form for a search, along with your payment, your search will be initiated as soon as possible. Most searches are completed within 3-6 months. Please be aware that the fee for conducting a search for your biological child is non-refundable, regardless of outcome.
